Co-trimoxazole is a mixture of the sulfonamide, Sulfamethoxazole and Trimethoprim and has been used in a wide variety of infections due to susceptible organisms, particularly those of the urinary, respiratory and gastro-intestinal tracts. The in vitro bactericidal action of co-trimoxazole is dependant on the presence of methionine, glycine and a purine which allows the inhibition of protein synthesis to be circumvented without antagonisms of the inhibition of DNA synthesis. The bacterial cells then grow without dividing and die.
When co-trimoxazole is administered by mouth, plasma concentrations of Trimethoprim and Sulfamethoxazole are generally around the optimal ratio of 1:20, although they may vary from 1:2 or 1:30 or more. In urine, the ratio may vary from 1:1 to 1:5 depending on the pH. About 50% Sulfamethoxazole is excreted in the urine in 24 hours, a large proportion of Sulfamethoxazole appears as inactive metabolite.

Respiratory Tract: Acute and chronic bronchitis, bronchiectasis, lobar and broncho-pneumonia, pneumocystis carinii, pneumonitis, otitis media and sinusitis.
Genito-Urinary Tract: Urethritis, cystitis, pyeliytis, prostatitis, and gonorrhoea.
Gastro-intestinal Tract: Typhoid and paratyphoid fevers, cholera and shigellosis.
Skin Infection: Pyoderma, abscesses and wound infections.
