COSATRIM®

CLINICAL PHARMACOLOGY:
Co-trimoxazole is a mixture of the sulfonamide, Sulfamethoxazole and Trimethoprim and has been used in a wide variety of infections due to susceptible organisms, particularly those of the urinary, respiratory and gastro-intestinal tracts. The in vitro bactericidal action of co-trimoxazole is dependant on the presence of methionine, glycine and a purine which allows the inhibition of protein synthesis to be circumvented without antagonisms of the inhibition of DNA synthesis. The bacterial cells then grow without dividing and die.

Pharmacokinetics:
When co-trimoxazole is administered by mouth, plasma concentrations of Trimethoprim and Sulfamethoxazole are generally around the optimal ratio of 1:20, although they may vary from 1:2 or 1:30 or more. In urine, the ratio may vary from 1:1 to 1:5 depending on the pH. About  50% Sulfamethoxazole is excreted in the urine in 24 hours, a large proportion of Sulfamethoxazole appears as inactive metabolite.

USES:
COSATRIM® is an antibacterial agent. It is effective against a wide range of Gram-positive and Gram-negative organisms. It is of value in the treatment of the following:-

Respiratory Tract: Acute and chronic bronchitis, bronchiectasis, lobar and broncho-pneumonia, pneumocystis carinii, pneumonitis, otitis media and sinusitis.
Genito-Urinary Tract: Urethritis, cystitis, pyeliytis, prostatitis, and gonorrhoea.
Gastro-intestinal Tract: Typhoid and paratyphoid fevers, cholera and shigellosis.
Skin Infection: Pyoderma, abscesses and wound infections.

COSATRIM®
Other bacterial Infections:
Acute and chronic osteomylitis, acute brucellosis septiceamias and other infections caused by other sensitive organisms.

DOSAGE  AND  ADMINISTRATION:
Take at regular intervals usually twice a day after meals. Complete the prescribed course unless otherwise directed.
COSATRIM®  Dispersible tablets to be dissolved or mixed  with water  before taking.
Adults and Children over 12 years: Standard Dosage: Two COSATRIM®  tablets or 1 COSATRIM®  DS twice daily.
For severe infections: Three COSATRIM® tablets or one and half COSATRIM®  DS twice daily.
Children: 6 – 12 years: COSATRIM®  1 tablet twice daily or COSATRIM® 240 Dispersible 2 tablets twice daily or COSATRIM® Suspension 10mL or two spoonfuls  twice daily,
6 months – 5 years: COSATRIM®  120 Dispersible 2 tablets twice daily or COSATRIM®  240 Dispersible  1 tablet twice daily or COSATRIM® suspension 5mL or one spoonful twice daily.
6 weeks – 5 months: COSATRIM® 120 Dispersible 1 tablet twice daily or COSATRIM® suspension 2.5mL or 1/2 spoonful twice daily.

CONTRA-INDICATIONS  AND  WARNINGS:
Precautions:

COSATRIM® should not be given to patients with a history of Sulfonamides, Trimethoprim or Sulfamethoxazole hypersensitivity. It should not be given in pregnancy and severe renal insufficiency.
COSATRIM®  should be discontinued if a skin rash appears. Care should be taken when treating the elderly because as a group, they are more susceptible to more reactions.
A folate supplement should be considered when treating potentially folate deficient patients or with prolonged dosage of COSATRIM®.

Adverse Effects:
Nausea, vomiting, diarrhoea and skin rashes can occur. Blood changes include thrombocytopenia. These are reversible on withdrawal of the drug.

Overdosage:
Treatment should consist of gastric lavage within an hour of ingestion. Increased fluid intake will increase the elimination of  sulfamethoxazole. General support measures are recommended.

Interactions:
The action of sulfonamides may be antagonised by p-aminobenzoic acid and compounds derived from it, particularly potassium aminobenzoate and the procaine group of local anaesthetics. Sulfamethoxazole and other sulfonamides may potentiate the effects of some drugs, such as oral anticoagulants methotrexate and phenytoin. Trimethoprim may increase serum concentrations and potentiate the effect of a number of drugs, including phenytoin, digoxin, and procainamide. Trimethoprim has been reported to reduce the renal excretion and increase blood concentrations of zidovudine, zalcitabine, and lamivudine. Trimethoprim and dapsone increase each other’s serum concentrations, whereas rifampicin may decrease trimethoprim concentrations. An increased risk of nephrotoxicity has been reported with the use of trimethoprim or co-trimoxazole and ciclosporin.

PHARMACEUTICAL  PRECAUTIONS:
Store in a dry place below 25°C.  Protect from light.  Keep all medicines out of the reach of children.
